胃腸管外科

    The Division of Gastroenterologic Surgery offers an education on patients requiring gastrointestinal surgery. Areas of expertise include gastric and small bowel disease, colorectal disease, proctology, surgical oncology.

    We have performed over 3,000 operations for gastric cancer for the last 15 years. The 5-year survival rates are 93% in stageⅠ, 78% in stageⅡ, 44% in stageⅢ, and 9% in stageⅣ. Laparoscopic surgery for gastric cancer was initiated in our department in 1993 for the first time in Korea. Colorectal cancer surgery has been done about 2,000 cases at the same time.

    Recently, several efforts have been made to minimize postoperative morbidity and mortality and to obtain a better postoperative quality of life without affecting survival rates. Subsequently, several limited surgical procedures for early gastric cancer have been developed, including radical gastrectomy with modified lymph node dissection, endoscopic mucosal resection (EMR), local resection, and pylorus-preserving gastrectomy (PPG). So, we have performed gastrectomy not uniformly but in case by case for quality of life and obtained favorable results.

    In addition, we interest in the pathogenesis of peritoneal or liver metastases of gastrointestinal tract cancer. We analyze both peripheral and drained blood of patients with gastrointestinal tract cancer at different stage to assess the presence of carcinoembryonic antigen (CEA) mRNA and cytokeratin20 (CK20) mRNA by reverse transcriptase-polymerase chain reaction (RT-PCR), which we used as an indicator for micrometastatic malignant cells and to evaluate the relation between the presence of cancer cells in blood according to the time course during a surgical procedure and metastases.
